This case report vividly illustrates the need for a valid risk-benefit ratio. A 9-year-old child underwent appendectomy under combined general and epidural anesthesia. A spinal cord injury occurred by the thoracic epidural injection performed in the anaesthetized patient.

There is a paucity of data; in the final meta-analysis only three trials could be included; there was no statistical significant difference in the proportions of infants having apnoea/bradycardia or oxygen desaturations.

But the evidence from these trials may no longer reflect current practice and is limited by small numbers and poor methodology.
