Anesth Analg 2003; 96:1547-1552. This describes a case series including 4298 lumbar epidural catheters inserted in anaesthetized adult surgical patients. This study questions the statement that patients have to be awake during block placement.
8. Craven PD, Badawi N, Henderson-Smart DJ, O'Brien M. Regional [spinal, epidural, caudal] versus general anaesthesia in preterm infants undergoing inguinal herniorrhaphy in early infancy. Cochrane Database Syst Rev 2003; 3:CD003669. This is a Cochrane database review on regional versus general anaesthesia in preterm infants undergoing inguinal hernia repair. There is a paucity of data; in the final meta-analysis only three trials could be included; there was no statistical significant difference in the proportions of infants having apnoea/bradycardia or oxygen desaturations.
9. Soh CR, Ng SB, Lim SL. Dorsal penile nerve block. Paediatr Anaesth 2003; 13:329-333. A large case series including 3909 penile blocks. There were no disastrous complications with long-term consequences; however, adverse events did occur in nine children.

12. Allan CY, Jacqueline PA, Shubhda JH. Caudal epidural block versus other methods of postoperative pain relief for circumcision in boys. Cochrane Database Syst Rev 2003; 2:CD003005. This is a Cochrane database review on caudal epidural block for circumcision compared with other methods of pain relief. Six trials compared caudal block with systemic medication; the need for rescue analgesia was reduced. But the evidence from these trials may no longer reflect current practice and is limited by small numbers and poor methodology.

14. Amory C, Mariscal A, Guyot E, et al. Is ilioinguinal/iliohypogastric nerve block always totally safe in children? Paediatr Anaesth 2003; 13:164-166. This case report illustrates that complications can occur even with the use of a correct technique and questions the statement that ilioinguinal nerve block is undoubtedly superior to a caudal block for analgesia after inguinal hernia repair.

17. Suraseranivongse S, Chowvanayotin S, Pirayavaraporn S, et al. Effect of bupivacaine with epinephrine wound instillation for pain relief after pediatric inguinal herniorrhaphy and hydrocelectomy. Reg Anesth Pain Med 2003; 28:24-28. This was the first trial to illustrate that wound instillation has a significant and clinically relevant analgesic effect compared with a placebo group in children undergoing inguinal hernia repair. The instilled fluid was allowed to remain for 20 or 60 s and was swabbed before wound closure.

20. Dadure C, Pirat P, Raux O, et al. Perioperative continuous peripheral nerve blocks with disposable infusion pumps in children: a prospective descriptive study. Anesth Analg 2003; 97:687-690. This describes a case series including 25 infants and children. The technical procedure is described; 20-gauge catheters are used. The authors concluded that continuous peripheral nerve infusion of 0.2% ropivacaine with disposable elastomeric pumps in children is effective.
