Data from 427 patients revealed that agitation in Alzheimer's disease is a frontal lobe syndrome and that is related to dementia severity.

A retrospective review of 100 patients diagnosed with FTD revealed that treatment with neuroleptic medication in 24 resulted in improvement in only nine.

Mild cognitive deterioration with subcortical features: prevalence, clinical characteristics, and association with cardiovascular risk factors in community-dwelling older persons (The InCHIANTI Study). J Am Geriatr Soc 2003; 51:1064-1071. Prevalence of cardiovascular and cerebrovascular risk factors was identified in a group of 556 patients aged 70-90. Risk factors for low cognitive performance (expressed as less than the 50th percentile in that population) with subcortical features included hypertension, atrial fibrillation, ECG changes and low HDL cholesterol.

This 12-month interim analysis of a 24-month study reports safety and cognitive benefit of treatment with 24 mg/day galantamine in patients with vascular dementia or combined Alzheimer's disease and vascular dementia.

Dement Geriatr Cogn Disord 2004; 17:29-34. This randomized, double-blind study in 285 patients with mixed vascular and Alzheimer's dementia showed that 24 mg/day galantamine halted cognitive decline at 6 months.

Efficacy and tolerability of donepezil in vascular dementia: positive results of a 24-week, multicenter, international, randomized, placebo-controlled clinical trial. Stroke 2003; 34:2323-2330. Nice randomized, placebo-controlled, double-blind, multicentre study in 603 patients, in which donepezil was tested in two doses. Several psychometric tests showed improvement after 24 weeks and few adverse events were reported.

A review article focusing on the different clinical features of Alzheimer's disease in increasing grades of severity. An overview of a number of exams to diagnose the disease completes the picture. A number of treatments are proposed and the practical approach applied to a patient affected with dementia scheduled for surgery is described.

The NMDA receptor may be involved in neuronal damage and the NMDA antagonist ketamine prevents postoperative neuronal dysfunction.

Paper doubting the value of the BIS in monitoring anaesthetic depth in patients with dementia, since it describes that baseline BIS value in these patients is significantly lower compared with controls.

A difference is reported by means of individual EEG analysis between normal elderly persons and mild Alzheimer's disease and vascular dementia patients. Power bands and frequency analysis help to discriminate between the latter two.

Quantified EEG and spectral analysis on the one hand and haemodynamical (total perfusion index) and morphological (brain atrophy index) monitoring both measured by MRI on the other hand lead to a correlation between neurophysiological and haemodynamical variables in Alzheimer's disease.
48. Balkan S, Yaras N, Mihci E, et al. Effect of donepezil on EEG spectral analysis in Alzheimer's disease. Acta Neurol Belg 2003; 103:164-169. The effectiveness of 10 mg/day donepezil in the treatment of Alzheimer's disease is reflected in the improvement of the EEG spectral analysis.
49. Farling P, Smith G. Anaesthesia for patients with Creutzfeldt-Jakob disease: a practical guide. Anaesthesia 2003; 58:627-629. Every aspect of organizing anaesthesia for patients with CJD is considered in this rich editorial. Although some questions might still remain, the authors' suggestions are more than welcomed and should be adopted worldwide whenever a patient suffering from or suspected to suffer from CJD is to be accepted in a medical facility.

This multicentre study looks at cognitive dysfunction at 1 week and 3 months in a group of patients (> 60 years) scheduled for minor surgery. The authors conclude that next to other factors, avoiding hospitalization might decrease postoperative brain dysfunction.

57. Rasmussen LS, Johnson T, Kuipers HM, et al. Does anaesthesia cause postoperative cognitive dysfunction? A randomised study of regional versus general anaesthesia in 438 elderly patients. Acta Anaesthesiol Scand 2003; 47:260-266. Cognitive dysfunction at 1 week and 3 months following surgery was studied and the influence of general or local anaesthesia was evaluated by several psychometric tests. There was no difference in incidence of postoperative cognitive dysfunction at 3 months between the two groups, indicating again no causative relationship between general anaesthesia and long-term postoperative cognitive dysfunction.
58. Lundstrom M, Edlund A, Bucht G, et al. Dementia after delirium in patients with femoral neck fractures. J Am Geriatr Soc 2003; 51:1002-1006. A 5-year follow-up study looking at the likelihood of developing dementia if perioperative delirium occurred during hospitalization for femoral neck fracture. Remaining lucid offered the best chance for not developing dementia.
59. Manfredi PL, Breuer B, Meier DE, Libow L. Pain assessment in elderly patients with severe dementia. J Pain Symptom Manage 2003; 25:48-52. Facial expressions were videotaped during dressing change for decubitus ulcer in normal and demented patients. The tapes were presented to naïve students and nurses. Reliability, sensitivity, specificity and positive and negative predictive value were reported.

This review deals with pain transmission systems and relates these systems to the specific neuropathology in Alzheimer's disease, vascular dementia and FTD. The different brain areas are mapped with their respective connections and a comparison is made with different types of dementia.
