The natural history of lower urinary tract dysfunction in men: the influence of detrusor underactivity on the outcome after transurethral resection of the prostate with a minimum 10-year urodynamic follow-up. BJU Int 2004; 93:745-750. This study has an impressive number of patients and 10 years minimum follow-up. It shows that symptoms and flowmetry are unreliable in detrusor underactivity, a condition in which TURP is unhelpful. The authors advocate routine preoperative urodynamic assessment for all men. Given the number of TURPs performed (e.g. 26 607 in England 2003-2004, Hospital Episode Statistics database), this would have enormous economic and workload implications.
