Use of regularly scheduled albuterol treatment in asthma: Genotype-stratified, randomised, placebo-controlled cross-over trial

Israel E, Chinchilli VM, Ford JG, et al. Use of regularly scheduled albuterol treatment in asthma: genotype-stratified, randomised, placebo-controlled cross-over trial. Lancet 2004; 364:1505-1512. This important study demonstrates that a genetic polymorphism in the β-adrenergic receptor alters clinical responsiveness to albuterol, leading to diminished asthma control. It also demonstrates that ipratropium bromide may be effective as a treatment for asthma exacerbations in patients with the described polymorphism. Characterization of a patient's genotype for specific genes should allow customization of treatment strategies (the pharmacogenetic approach).
