40. Alboni P, Botto GL, Baldi N, et al. Outpatient treatment of recent-onset atrial fibrillation with the 'pill-in-the-pocket' approach. N Engl J Med 2004; 351: 2384-2391. This is a well-designed study of a new strategy consisting of self-administration of flecainide or propafenone for terminating symptomatic paroxysmal atrial fibrillation of recent onset outside the hospital.

The authors present a retrospective analyses of the AFFIRM database to investigate whether exposure to angiotensin II receptor blockers or angiotensin-converting enzyme inhibitors reduce the recurrence rate of atrial fibrillation.

44. Dernellis J, Panaretou M. Relationship between C-reactive protein concentrations during glucocorticoid therapy and recurrent atrial fibrillation. Eur Heart J 2004; 25:1100-1107. This interesting clinical study shows that methylprednisolone successfully prevents recurrent and permanent atrial fibrillation when administered for treatment of the first symptomatic and persistent episode of atrial fibrillation in patients with elevated CRP.

48. Nademanee K, McKenzie J, Kosar E, et al. A new approach for catheter ablation of atrial fibrillation: mapping of the electrophysiologic substrate. J Am Coll Cardiol 2004; 43:2044-2053. This innovative approach to atrial fibrillation ablation, targets areas of complex fractionated atrial electrograms, that are known to correlate with areas of slowed conduction and pivot points of reentrant wavelets. At the one-year follow-up, 91% of the patients were free of arrhythmia.

This interesting study from an experienced center reports beneficial effects associated with restoration and maintenance of sinus rhythm without antiarrhythmic drugs in patients with congestive heart failure.
50. Pappone C, Oral H, Santinelli V, et al. Atrio-esophageal fistula as a complication of percutaneous transcatheter ablation of atrial fibrillation. Circulation 2004; 109:2724-2726. Case reports of one of the most serious complications related to catheter ablation along the posterior left atrium are presented.
51. Gage BF, Walraven C, Pearce L, et al. Selecting patients with atrial fibrillation for anticoagulation. Circulation 2004; 110:2287-2292. Here is an interesting study that compares five stroke-risk stratification schemes used by clinicians to decide about antithrombotic therapy.
52. Waldo AL. New possibilities in anticoagulant management of atrial fibrillation. Rev Cardiovasc Med 2004; 5(suppl 5):S30-S38. The author presents an excellent review of current and future anticoagulation strategies for atrial fibrillation.
