This article describes a meta-analysis of three randomized controlled trials on the use of prophylactic intravenous fluconazole in premature infants in the prevention of invasive fungal disease and the reduction of mortality. The authors concluded that the use of prophylactic intravenous fluconazole may reduce mortality before hospital discharge but that further studies were indicated.

Pediatrics 2004; 113:e608-e616. This excellent study suggests that sodium supplementation during amphotericin B therapy may reduce the incidence of nephrotoxicity. Neonates with amphotericin B-induced nephrotoxicity had lower mean sodium intakes and a higher incidence of hyponatremia.

This small case series involved nine preterm neonates and one full-term neonate with candidemia in whom amphotericin B therapy was unsuccessful. They were subsequently treated successfully with caspofungin. No adverse events were observed, suggesting that caspofungin is an alternative therapy for invasive fungal disease in neonates.
