This study determined that in non-selected hospitalized patients, malnutrition, assessed by the NRI, was an independent factor in nosocomial infection. Early screening for malnutrition may therefore be helpful to reduce the high prevalence of nosocomial infection.

The identification of potential risk in obese patients must go beyond the measurement of such indices as BMI and percentage of weight loss in order to determine nutritional risk in obese individuals.

J Parenter Enter Nutr 2004; 28:99-104. This study found that severe nutritional depletion by the NRI was significantly associated with LOS greater than 11 days. Albumin alone underestimated the nutritional risk. It furthermore found that malnutrition is pre-existing to hospital admission. Nutritional screening should take place at hospital admission to determine nutritional risk and the risk of longer LOS.

Clin Nutr 2004; 23:1016-1024. This study showed that anthropometry and SGA techniques were not concordant. Whereas with SGA, malnutrition was detected in patients with normal and high BMI, with anthropometry overnuirition was diagnosed. SGA seems to be more accurate than anthropometry to anticipate hospital LOS and readmission rates.

This study found that the NRS-2000 was unspecific but highly sensitive (> or = 95%) and the SGA was highly sensitive (100%) and specific (69%). The NRS-2000 was devised to direct nutritional intervention, whereas recent unintentional weight loss greater than 10% is a simple method to identify nutritional risk and SGA identified high-risk/undernourished patients

This study evaluated nutritional screening tools (SGA, NRI, anthropometric measurements, and laboratory data) and found that the two screening tools correlated with each other with respect to age, LOS, and anthropometric and laboratory data in hospitalized patients. Therefore, these two techniques can be used for nutritional assessment in hospitalized patients.

    • Kyle UG, Pirlich M, Schuetz T, et al. Increased length of hospital stay in underweight and overweight patients at hospital admission: a controlled population study. Clin Nutr 2005; 24:133-142. This study noted that both the depletion of lean tissue and excess of fat mass negatively affect LOS. Furthermore, it found that excess fat mass reduced the sensitivity of BMI to detect nutritional depletion. The higher likelihood of having a low FFM index than having a low FFM index/high fat mass index with recent weight loss suggests that a high fat mass index may exert a protective effect on body composition with weight loss.
