This randomized controlled trial compared a designated outreach service in London with standard care for people with early psychosis (age 16-40 years). The designated service was associated with fewer readmissions to hospital and better service engagement.

This is an important pragmatic evaluation of the impact of early psychosis services in Australia, particularly restructuring services and training staff. The paper has valuable lessons for applied service evaluation.

J Abnorm Child Psychol 2004; 32:249-261. This interesting longitudinal study was conducted from the third trimester of pregnancy through the first year of parenthood in 203 adolescent mothers. The relation between mothers' own history of physical maltreatment and subsequent difficulties with their infant was mediated by the relationship with their parent and with the feelings they associated with motherhood before childbirth.
44. Ekeus C, Christensson K, Hjern A. Unintentional and violent injuries among pre-school children of teenage mothers in Sweden: a national cohort study. J Epidemiol Community Health 2004; 58:680-685. A national cohort of 800 000 children was followed up 7 years from birth. Children of teenage mothers (18 years or under) had higher relative risks for hospital admission for unintentional and violent injuries, as well as deaths following violent injuries, as compared with children of older mothers. The risks remained after adjusting for socioeconomic deprivation, and parental substance abuse and psychiatric illness.
45. Milan S, Ickovics J, Kershaw T, et al. Prevalence, course, and predictors of emotional distress in pregnant and parenting adolescents. J Consult Clin Psychol 2004; 72:328-340. This study examined factors associated with emotional distress in pregnant (n = 203) and nulliparous (n = 188) adolescents from disadvantaged communities over 18 months. Both groups demonstrated higher levels of emotional distress than expected in the general population, with history of physical maltreatment and partner support playing a more significant role in pregnant adolescents.

47. Thomas D, Looney S. Effectiveness of a comprehensive psycho educational intervention with pregnant and parenting adolescents. J Child Adolesc Psychiatr Nurs 2004; 17:66-77. This paper reports an evaluation of a psychoeducational intervention for 41 pregnant teenagers and young mothers in residential settings or attending school. This was found to help change parenting attitudes and beliefs.
