This is a useful summary of the current quality evidence for this intervention. To date, there are only data on a relatively small number of patient interventions. This has limited our ability to make informed judgments about timing of the intervention and the likely benefit and risk.

Pilot dose-escalation study of caffeine plus ethanol (caffeinol) in acute ischemic stroke
-
Piriyawat P, Labiche LA, Burgin WS, et al.: Pilot dose-escalation study of caffeine plus ethanol (caffeinol) in acute ischemic stroke. Stroke 2003, 34:1242-1245. This is an interesting and well-conducted phase I trial. Target plasma levels were achieved easily for both agents. Positive results from an outcome trial are needed, but I believe that this combination is already popular with my northern European aging population (personal observation).
