In this prospective cohort study, the short-term outcome of children with asthma attending the emergency department is worse than expected, particularly in children under 2 years of age. However, in children older than 2 years, an adequate medical follow-up and the use of inhaled corticosteroids in the maintenance treatment of asthma favored a prompt recovery.

Interesting review of publications contained in the Cochrane Library of the role of systemic and inhaled corticosteroids in the treatment of asthma. It is suggested that an emergency department visit is a great opportunity to review the patient's asthma maintenance treatment, and there is some evidence to support the recommendation of the combined use of systemic and inhaled steroids at the time of discharge.

This systematic review concludes that a short course (5-10 days) of corticosteroids (oral or intramuscular) following assessment for an acute exacerbation of asthma significantly reduces the number of relapses to additional care and decreases β-agonist use without an apparent increase in side effects.

A review of relevant articles of inhaled corticosteroid use in pediatric asthma consistently shows that inhaled steroid therapy is the most favorable treatment option with regard to safety and efficacy in infants and young children with persistent asthma. However, because budesonide inhalation suspension as a nebulized formulation is the only corticosteroid available in the United States, this agent has been extensively studied in children younger than 5 years.

This study examined the effectiveness of salbutamol delivered via a MDI with spacer versus a nebulizer for acute asthma treatment in the pediatric emergency department. It is shown that the numbers of children who required a stay in the observation unit, admission to the hospital, or returned for medical care were similar for both modes of salbutamol administration.
