Crucial role of sodium channel fast inactivation in muscle fibre inexcitability in a rat model of critical illness myopathy

Rich MM, Pinter MJ. Crucial role of sodium channel fast inactivation in muscle fibre inexcitability in a rat model of critical illness myopathy. J Physiol 2003; 547:555-566. In an experimental model of denervated rat muscle fibers and high-dose steroid, the authors demonstrate that a shift in the voltage dependence of sodium channel fast inactivation to more negative potentials has a crucial role to cause muscle inexcitability.

Filatov GN, Rich MM. Hyperpolarized shifts in the voltage dependence of fast inactivation of Nav1.4 and Nav1.5 in a rat model of critical illness myopathy. J Physiol 2004; 559:813-820. In an experimental model of denervated rat muscle fibers and high-dose steroid, the authors demonstrate an increased density of embryonic sodium channel isoforms. Increased density did not correlate with muscle unexcitability, which was explained by the hyperpolarized shift in both embryonic and adult sodium channel isoforms.

Di Giovanni S, Molon A, Broccolini A, et al. Constitutive activation of MAPK cascade in acute quadriplegic myopathy. Ann Neurol 2004; 55:195-206. Activation of the TGF-beta/MAPK pathway is specific of critical illness myopathy. It is not seen in neurogenic atrophy, or in inflammatory myopathies.

Electrophoretic determination of the myosin/actin ratio in the diagnosis of critical illness myopathy
-
Stibler H, Edstrom L, Ahlbeck K, et al. Electrophoretic determination of the myosin/actin ratio in the diagnosis of critical illness myopathy. Intensive Care Med 2003; 29:1515-1527. A net reduction of the myosin/actin ratio seems specific of critical illness myopathy, and can be rapidly (24 hours) diagnosed using an electrophoretic method.
