This well designed, single-centre cohort study of birth defects in offspring of women with epilepsy found a higher malformation rate among infants exposed to antiepileptic drugs in utero than in offspring of untreated women with epilepsy, but it did not reveal any differences between drugs.

The conclusion was that the majority of studies were of limited quality and that there was little evidence about which drugs carry more risk than others.

This is a thorough re-evaluation of a previous retrospective cohort suggesting that frequent tonic-clonic seizures during pregnancy and exposure to valproic acid in utero are associated with lower verbal IQ in the children. The retrospective design, low rate of participation and small numbers call for caution in Interpretation of the data.

This well designed prospective study of long-term postnatal effects of exposure to AEDs in utero indicates lack of adverse effects of carbamazepine. Only few exposures to valproic acid are included.

IQ at 7 years of age was similar among children exposed to phenobarbital compared with placebo.
