This study confirms the importance of tumour-free margins in the wide local excision. The advice is to perform the local excision with margins of 2 cm. Moreover, the study warns that attention should be paid to the higher regional recurrence rate since the introduction of separate incisions.

The first series of nine patients with vulvar melanoma who underwent the sentinel lymph node procedure as 'standard care'. Two groin recurrences after negative sentinel lymph node were identified, probably in-transit metastases. The pros and cons of this technique in patients with vulvar melanoma are discussed.
