The prevalence of PHG was 80%. PHG was stable in 29%, deteriorated in 23%, improved in 23%, and fluctuated with time in 25%. Acute bleeding from gastropathy occurred in 8 of 315 patients (2.5%). The bleeding-related mortality rate was 12.5%. Chronic bleeding occurred in 34 patients (10.8%).

This study examined the relation between PHG in patients with PH due to cirrhosis, noncirrhotic patients with extrahepatic portal vein obstruction, cirrhotic patients with extrahepatic and 57 control subjects. Snakeskin, scarlatina rash, diffuse hyperemia, and diffuse bleeding were frequent endoscopic gastric findings in cirrhotic patients. These findings were seen less frequently in noncirrhotic patients with portal hypertension caused by portal vein obstruction than in cirrhotic patients (P<0.0001). The highest incidence was seen in cirrhotic patients with portal vein obstruction (P<0.001). Positive correlations existed among the endoscopic findings, the clinical estimate of the cirrhosis severity (Child-Pugh grade), and the size and appearance of esophageal varices.

    • McCormack TT, Sims J, Eyre-Brook I, et al.: Gastric lesions in portal hypertension: inflammatory gastritis or congestive gastropathy? Gut 1985, 26:1226-1232. This is excellent study was the first to propose a standardized system for grading PHG. The authors' proposed scoring system is likely the most commonly used system in the present literature. They assessed the incidence and natural history of PHG in a series of 127 patients with PH of various causes. Gastritis was observed endoscopically in 65 patients (51%) and was of two main types. Twenty-eight patients had severe or persistent gastritis, which caused clinically significant bleeding on 80 occasions and accounted for 25% of the bleeds from all sources.

The authors showed a high incidence of PHG in patients being assessed for liver transplantation: 73% of patients had esophageal varices, 62% PHG, and 16% gastric had varices. Excluding varices and PHG, endoscopic findings in the upper gastrointestinal tract (n = 120) included 13% esophagitis/ulcers, 7.5% gastritis, 8% duodenitis, 2% Barrett esophagus, 3% duodenal ulcers, and 2% gastric ulcers. Findings in the lower gastrointestinal tract (n = 71) included 21% adenomatous polyps, 7% rectal varices, and 3% colopathy.
