Resuscitation 2001, 51:301-308. Results showed that combination drug therapy before countershock produced far superior rates of ROSC and 1-hour survival compared with single-drug therapy and standard advanced care life support protocols. This was a porcine model of prolonged ventricular fibrillation. Interestingly, antioxidants were a major component of this cocktail and suggest a potential role of ROS in electrical arrhythmias and defibrillation thresholds.

Ventricular fibrillation and ventricular tachycardia are potentially life-threatening complications of coronary bypass. This study shows that preconditioning significantly reduced postoperative ventricular fibrillation/ventricular tachycardia in patients who underwent coronary artery bypass graft. Ventricular tachycardia during early reperfusion and 24 hours after reperfusion was suppressed and is suggestive of both the first and second windows of protection. This is one of the largest clinical studies of preconditioning.

Modifications by preconditioning of connexin43 may provide important insights into antiarrhythmic effects.

This study shows that an inhibitor of the NHE prevents the activation of calpain after reperfusion, suggesting another possible mechanism of protection for NHE inhibitors. Proton accumulation during ischemia is corrected by the NHE, whereby the sodium-calcium exchange leads to a build up of calcium and subsequent activation of calpain. Calpain mediates mitochondrial dysfunction, release of proapoptotic factors, and cell death. In the context of other studies suggesting that NHE is a target for preconditioning and its inhibition can improve outcome in cardiac arrest, this study explores important consequences of NHE inhibitor protection.

The results of this study implicate the generation of nonlethal levels of ROS as the triggering mechanism of volatile anesthetic-induced protection. When the antioxidants N-2-mercaptopropionyl glycine and N-acetylcysteine were coadministered with isoflurane, they abolished the infarct-sparing effect of the anesthetic. This study demonstrates a possible means of triggering preconditioning protection via a volatile anesthetic.
