A rigorous clinical trial with good maintenance of blinding and use of detailed protocols. The results are credible and of importance. They identify the large impact of timing of interventions and set the stage for more attention to this variable in future clinical trials and clinical care.

A double-blinded, randomized trial under challenging circumstances was completed successfully. The results provide credible evidence of efficacy of hyperbaric oxygen therapy in reducing cognitive deficits after carbon monoxide poisoning. This is the first double-blinded, randomized, single-center trial in hyperbaric medicine. It raises the bar for future studies in this and other critical care fields.
