Early goal-directed therapy in the treatment of severe sepsis and septic shock
-
Rivers E, Nguyen B, Havstad S, et al.: Early goal-directed therapy in the treatment of severe sepsis and septic shock. N Engl J Med 2001, 345:1368-1377. A rigorous clinical trial with good maintenance of blinding and use of detailed protocols. The results are credible and of importance. They identify the large impact of timing of interventions and set the stage for more attention to this variable in future clinical trials and clinical care.

Hyperbaric oxygen for acute carbon monoxide poisoning
-
Weaver LK, Hopkins RO, Chan KJ, et al.: Hyperbaric oxygen for acute carbon monoxide poisoning. N Engl J Med 2002, 347:1057-1067. A double-blinded, randomized trial under challenging circumstances was completed successfully. The results provide credible evidence of efficacy of hyperbaric oxygen therapy in reducing cognitive deficits after carbon monoxide poisoning. This is the first double-blinded, randomized, single-center trial in hyperbaric medicine. It raises the bar for future studies in this and other critical care fields.
