Vitamin E therapy significantly improved haemodynamic function in aortic-banded animals.

2α in pericardial fluid of patients with heart failure: A potential role for in vivo oxidant stress in ventricular dilatation and progression to heart failure
-
2α (a marker of oxidative stress) were measured in 51 patients undergoing cardiac surgery and found to be significantly elevated. These levels were also correlated with the degree of left ventricular (LV) dysfunction. This was the most convincing demonstration in the literature of increased oxidative stress in the failing human heart, using a more reliable marker than previous studies and correlating it with LV dysfunction.

Using electron spin resonance spectroscopy in a rapid pacing canine model of heart failure, this was the first direct demonstration of an increase in reactive oxygen species (ROS) production by the failing myocardium. A previous study by the same group identified the mitochondria as being an important source of ROS within the myocyte.

This study showed a four-fold increase in xanthine oxidase activity in failing myocardium. It also demonstrated an interesting effect of the xanthine oxidase inhibitor, allopurinol, which increased contractility in the failing hearts whilst simultaneously decreasing myocardial oxygen consumption. This effect on myocardial efficiency has not been seen with any other inotropic agent.

In 15 patients with New York Heart Association class III heart failure, the effects of intrabrachial infusion of vitamin C were assessed and shown to improve flow-dependent vasodilatation. Four weeks of oral vitamin C therapy also improved endothelial function in these patients.
