The roles of insulin resistance and insulin secretion in the pathogenesis of glucose intolerance in polycystic ovary syndrome (PCOS) were evaluated in obese adolescents with glucose intolerance. Multiple metabolic abnormalities of insulin action were noted and an absence of nocturnal dipping in blood pressure, which are precursors of type 2 diabetes and present earlier in the course of PCOS then previously suspected.

This was an important follow-up study in a thin Dutch PCOS population investigating the prevalence of diabetes mellitus, hypertension, and cardiac complaints in a Dutch population with polycystic ovarian syndrome (PCOS) compared with theprevalence of these conditions in the Dutch female population. A slight but increased risk was identified in women with PCOS in their 40's.

This is the largest study to date of the conversion rates to impaired glucose tolerance or to type 2 diabetes in a cohort of 67 women with PCOS. 17% of the women with normal glucose tolerance progressed and 54% of the women with IGT progressed.
