IL-8 was found to be related to pain intensity in patients with FM (r=0.35; P≤0.01). The authors concluded that IL-8 may play an important role in the occurrence of pain in FM.

This may explain the response to nonsteroidal anti-inflammatory drug therapy in a subset of FM patients.

J Musculoske Pain 2001, 9(suppl 5):77. • The expression of delta opioid receptor [81.0 fold (P=0.001)] and kappa opioid receptor [13.6 fold (P=0.009)] mRNA was significantly increased in FM skin compared with that of healthy controls. No significant differences could be detected in muscle tissues. The data suggest that opioid antinociception can be initiated by activation of opioid receptors present outside of the central nervous system, which could be the result of severe and persistent pain.

Arthritis Rheum 2002, 46:1344-1350. •• Three new findings are reported: (1) FM patients have a reduced growth hormone response to exercise, (2) pyridostigmine reverses this impaired response, and (3) defective growth hormone secretion in FM can occur in patients with normal insulin-like growth factor-1 levels. Because pyridostigmine is known to reduce somatostatin tone, it is surmised that the defective growth hormone response to exercise in FM patients probably results from increased levels of scmatostatin, a hypothalamic hormone that inhibits growth hormone secretion.
