Critical care in the emergency department: A physiologic assessment and outcome evaluation
-
Nguyen HB, Rivers EP, Havstad S, et al.: Critical care in the emergency department: a physiologic assessment and outcome evaluation. Acad Emerg Med 2000, 7:1354-1361. This study showed that care provided during the emergency department stay for critically ill patients significantly impacts the progression of organ failure and mortality. Although this period is brief compared with the total length of hospitalization, physiologic determinants of outcome may be established before ICU admission. This study emphasizes the importance of emergency department intervention and suggests that unique physiologic assessment methodologies should be developed to examine the quality of patient care, improve the accuracy of prognostic decisions, and objectively measure the impact of clinical interventions and pathways in the emergency department setting.

The golden hour and the silver day: Detection and correction of occult hypoperfusion within 24 hours improves outcome from major trauma
-
Blow O, Magliore L, Claridge JA, et al.: The golden hour and the silver day: detection and correction of occult hypoperfusion within 24 hours improves outcome from major trauma. J Trauma 1999, 47:964-969. The significance of occult hypoperfusion in the development of respiratory complications, multisystem organ failure, and death and the effect of rapid identification and correction of occult hypoperfusion in the severely injured trauma patient was investigated, Initial lactic acidosis is associated with lower cardiac performance and higher morbidity and mortality, Persistent occult hypoperfusion is associated with higher rates of respiratory complications, multisystem organ failure, and death after severe trauma. Early identification and aggressive resuscitation aimed at correcting continued elevation in serum lactate concentration improves survival and reduces complications in severely injured trauma patients.

Septic shock: An analysis of outcomes for patients with onset on hospital wards versus intensive care units
-
Lundberg JS, Perl TM, Wiblin T, et al.: Septic shock: an analysis of outcomes for patients with onset on hospital wards versus intensive care units, Crit Care Med 1998, 26:1020-1024. This study suggests that for patients with septic shock on general hospital wards, there were clinically important delays in the transfer of patients to the ICU, receipt of intravenous fluid boluses, and receipt of inotropic agents that may affect outcomes.

Early goal-directed therapy in the treatment of severe sepsis and septic shock
-
Rivers E, Nguyen B, Havstad S, et al.: Early goal-directed therapy in the treatment of severe sepsis and septic shock. N Engl J Med 2001, 345:1368-1377. This approach involves adjustments of cardiac preload, afterload, and contractility to balance oxygen delivery with oxygen demand. The purpose of this study was to evaluate the efficacy of early goal-directed therapy before admission to the ICU. Early goal-directed therapy provides significant benefits with respect to outcome in patients with severe sepsis and septic shock.

Effects of a medical emergency team on reduction of incidence of and mortality from unexpected cardiac arrests in hospital: Preliminary study
-
Buist MD, Moore GE, Bernard SA, et al.: Effects of a medical emergency team on reduction of incidence of and mortality from unexpected cardiac arrests in hospital: Preliminary study. BMJ 2002, 324:387-390. This study was conducted to determine whether earlier clinical intervention by a medical emergency team prompted by clinical instability in a patient could reduce the incidence of and mortality from unexpected cardiac arrest in the hospital. The investigators found that early intervention by a medical emergency team does significantly reduce the incidence of and mortality from unexpected cardiac arrest in clinically unstable inpatients.
