    • Nguyen HB, Rivers EP, Havstad S, et al.: Critical care in the emergency department: a physiologic assessment and outcome evaluation. Acad Emerg Med 2000, 7:1354-1361. This study showed that care provided during the emergency department stay for critically ill patients significantly impacts the progression of organ failure and mortality. Although this period is brief compared with the total length of hospitalization, physiologic determinants of outcome may be established before ICU admission. This study emphasizes the importance of emergency department intervention and suggests that unique physiologic assessment methodologies should be developed to examine the quality of patient care, improve the accuracy of prognostic decisions, and objectively measure the impact of clinical interventions and pathways in the emergency department setting.

J Trauma 1999, 47:964-969. The significance of occult hypoperfusion in the development of respiratory complications, multisystem organ failure, and death and the effect of rapid identification and correction of occult hypoperfusion in the severely injured trauma patient was investigated, Initial lactic acidosis is associated with lower cardiac performance and higher morbidity and mortality, Persistent occult hypoperfusion is associated with higher rates of respiratory complications, multisystem organ failure, and death after severe trauma. Early identification and aggressive resuscitation aimed at correcting continued elevation in serum lactate concentration improves survival and reduces complications in severely injured trauma patients.

This approach involves adjustments of cardiac preload, afterload, and contractility to balance oxygen delivery with oxygen demand. The purpose of this study was to evaluate the efficacy of early goal-directed therapy before admission to the ICU. Early goal-directed therapy provides significant benefits with respect to outcome in patients with severe sepsis and septic shock.

BMJ 2002, 324:387-390. This study was conducted to determine whether earlier clinical intervention by a medical emergency team prompted by clinical instability in a patient could reduce the incidence of and mortality from unexpected cardiac arrest in the hospital. The investigators found that early intervention by a medical emergency team does significantly reduce the incidence of and mortality from unexpected cardiac arrest in clinically unstable inpatients.
