Spinal immobilization in trauma patients: Is it really necessary?

Neurosurgery 2002, 50(suppl):S7-S17. This is the first chapter in the latest Guidelines for the Management of Acute Cervical Spine Injuries from the Congress of Neurological Surgeons and the American Association of Neurological Surgeons, which appeared as a supplement in Neurosurgery. This is a great resource for anyone taking care of trauma patients. Each chapter begins with a summary of the evidence, followed by a review of the literature. Unfortunately, we disagree with the conclusions of this particular chapter. There is enough evidence that selective immobilization based on clinical criteria rather than mechanism of injury should at least be an option for prehospital management.
