note

Ahmed AR: Intravenous immunoglobulin therapy in the treatment of patients with mephigus vulgaris unresponsive to conventional immunosuppressive treatment. J Am Acad Dermatol (2001) 45:679-690. In patients with PV who do not respond to conventional immunosuppressants, IVIg appears to be an effective treatment alternative. Its early use is of significant benefit in patients who may experience life-threatening complications from immunosuppression. IVIg is effective as monotherapy.

Intravenous immunoglobulin therapy for patients with bullous pemphigoid unresponsive to conventional immunosuppressive treatment
-
note
-
Ahmed AR: Intravenous immunoglobulin therapy for patients with bullous pemphigoid unresponsive to conventional immunosuppressive treatment. J Am Acad Dermatol (2001) 45:825-835. IVIg appears to be an effective alternative in treating patients with severe BP whose disease is unresponsive to conventional therapy. IVIg may be particularly useful, if treatment is begun early, in patients who are at risk of experiencing serious or potentially fatal side effects from conventional immunosuppressive therapy. After clinical control is achieved, IVIg therapy should be gradually withdrawn and not abruptly discontined.

Dalakas MC: Controlled studies with high-dose intravenous immunoglobulin in the treatment of dermatomyositis, inclusion body myositis, and polymyositis. Neurology (1998) 51:S37-S45. High-dose IVIg has been tried in controlled clinical trials in patients with DM and IBM but not with PM. In patients with DM that is resistant or partially responsive to coventional therapies, IVIg was very effective. The treated patients experienced dramatic improvement not only in muscle strength but also of their skin rash.

Inhibition of toxic epidermal necrolysis by blockade of CD95 with human intravenous immunoglobulin
-
note

Viard I, Wehrli P, Bullani R, Schneider P, Holler N, Salomon D, Hunziker T, Saurat JH, Tschopp J, French LE: Inhibition of toxic epidermal necrolysis by blockade of CD95 with human intravenous immunoglobulin. Science (1998) 282:490-493. In a pilot study, ten consecutive individuals with clinically and histologically confirmed TEN were treated with IVIg; Disease progression was rapidly reversed and the outcome was favorable in all cases. Thus, Fas-FasL interactions are directly involved in the epidermal necrolysis of TEN, and IVIg may be an effective treatment.
