This is a detailed assessment of the 113 participants in the single randomized controlled trial of hyperventilation for traumatic brain injury. For hyperventilation alone, the relative risk (RR) for death or severe disability was 1.14 (95% CI 0.82 to 1.58). The RR for death or severe disability in the hyperventilation plus tromethamine group was 0.87 (95% CI 0.58 to 1.28). The authors conclude that the available data are inadequate to assess potential benefits or harm that might result from hyperventilation in severe head injury.

J Neurosurg 2000, 92:7-13. This is a prospective study of moderate hyperventilation, induced early after traumatic brain injury, and its effect on cerebral metabolism. Nine adults were studied, and the data were compared with 10 age-matched control participants. During hyperventilation, global cerebral blood flow, cerebral blood volume, and cerebral venous oxygen content all decreased; but, the cerebral metabolic rate for oxygen remained unchanged. The authors concluded that early, brief, moderate hyperventilation does not impair global cerebral metabolism in patients with severe traumatic brain injury.
