Long-term follow up in the largest trial of patients with multivessel disease randomized to CABG versus PTCA showed a significant overall survival benefit for CABG at 7 years (84.4 vs 80.9%, P = 0.043). This benefit was entirely attributable to improved survival in diabetic patients (76.4 with CABG vs 55.7% with PTCA, P = 0.0011), with no difference in survival among patients without treated diabetes.

This study suggests that the survival benefit seen with CABG in diabetic patients may be significantly related to an improvement in subsequent mortality with myocardial infarction. Among patients with multivessel disease eligible for the BARI trial, diabetic patients had significantly higher rates of death and spontaneous Q-wave MI at 5 years compared with non-diabetic patients. Previous CABG (as initial revascularization or subsequent procedure) dramatically reduced the high mortality after spontaneous Q-wave myocardial infarction in diabetic patients (relative risk 0.09, P< .001) but not in non-diabetic patients (P = .26). A smaller protective effect of CABG on mortality was also seen in diabetic patients who did not have spontaneous Q-wave myocardial infarction (relative risk 0.65).
