Twenty-eight patients undergoing elective abdominal surgery were randomized to receive standard TPN or glutamine-enhanced TPN for 5 days postoperatively. Patients receiving glutamine-enhanced peptide had improved nitrogen balances and lymphocyte recovery, as indicated by a shortened duration of hospital stay. The small sample poses significant limitations to this study.

One hundred and sixty-six consecutive patients undergoing curative surgery for gastric or pancreatic cancer were randomized to receive a standard enterai formula, IEF or TPN perioperatively. The enriched enterai group demonstrated a lower severity of infection and duration of hospital stay in certain subgroups. Early enterai feeding was shown to be a suitable alternative to TPN after major abdominal surgery.

Two hundred and six patients with neoplasm of colorectum, stomach, or pancreas were randomized to receive either a standard enterai formula or an immune-enhanced version of the same formula for 7 consecutive days before surgery. Perioperative administration of a supplemented enterai formula significantly reduced postoperative infections and duration of hospital stay in patients undergoing surgery for cancer. This is a large well designed study.

These authors concluded that the provision of an IEF decreases infectious complications and duration of hospital stay for both cancer patients and critically ill patients This meta-analysis has important implications for the care of this patient population.

This study sought to evaluate the reliability of perioperative serum albumin levels in predicting surgical outcome. Forty-four Veterans' Affairs institutions were involved, and patients undergoing 54 215 major noncardiac operations were analyzed. A decrease in serum albumin from 46 to < 21 g/l was associated with an increase in mortality rates from less than 1 to 29% and in morbidity rates from 10 to 65%.
