JAMA 1998; 280:605-613. •• This is an outstanding study, the first randomized controlled trial of HRT/placebo in women with coronary disease. The outcome was neutral. There was an increased risk of an event in the first year, which was reduced in the last 3 years. The results do not confirm the protective effect of HRT in observational studies.

Those most at risk benefited most. Significantly reduced cardiovascular mortality, and no increase in cancer morbidity and mortality was found.

The first-pass effect of oestrogen results in a reduction of serum insulin-like growth factor type 1 and increases circulating growth hormone as well as growth hormone-binding protein levels. Different effects on fat mass and lean body mass were observed, with an average increase of fat mass of 1.2 kg and a corresponding decrease in lean body mass. No such effect was found with the transdermal route.

The mean lipoprotein (a) concentration was reduced by 20% as a result of a decrease of the mean production rate of apo(a), but the mean fractional catabolic rate of apo(a) was unaltered by oestrogen replacement.
