There was an increased risk of an event in the first year, which was reduced in the last 3 years. The results do not confirm the protective effect of HRT in observational studies.

Science 1999; 285:1929-1931. •• This electrophysiological and binding study showed that the direct interaction of estradiol with an external binding site in the presence of a beta subunit mediated the effect of oestrogen on Maxi-K channels in smooth muscle, an effect not found in skeletal muscle, which is devoid of beta subunit.

J Clin Invest 1998; 102:1035-1040. •• One of the few scientific papers investigating the differences in metabolic effects between the routes of administration of HRT; oral and transdermal oestrogen were administered as a cross-over design. The first-pass effect of oestrogen results in a reduction of serum insulin-like growth factor type 1 and increases circulating growth hormone as well as growth hormone-binding protein levels. Different effects on fat mass and lean body mass were observed, with an average increase of fat mass of 1.2 kg and a corresponding decrease in lean body mass. No such effect was found with the transdermal route.

J Clin Endocrinol Metab 1998; 83:3267-3276. •• Cold isotope was employed for the labelling of both apo(a) and apoB100 in lipoprotein (a) before and after oestrogen treatment. The mean lipoprotein (a) concentration was reduced by 20% as a result of a decrease of the mean production rate of apo(a), but the mean fractional catabolic rate of apo(a) was unaltered by oestrogen replacement.
