Palivizumab, a humanized respiratory syncytial virus monoclonal antibody, reduces hospitalization from respiratory syncytial virus infection in high-risk infants
-
The IMpact-RSV Study Group: Palivizumab, a humanized respiratory syncytial virus monoclonal antibody, reduces hospitalization from respiratory syncytial virus infection in high-risk infants. Pediatrics 1998, 102:531-537. This report of a randomized, double-blind, placebo-controlled trial involving 1502 children at high risk for RSV is the first demonstration of the clinical efficacy of a monoclonal antibody in human infectious disease. The regime of monthly intramuscular injections represents an important advance in RSV prophylaxis in high risk infants.

N Engl J Med 1997, 337:874-880. Report of two parallel, randomized, double-blind, placebo-controlled, multicenter trials conducted in North America and Europe, involving 417 patients (only 262 of whom had confirmed influenza infection). Zanamavir given intranasally and by inhalation clearly demonstrated benefit only if treatment began within 30 hours of symptom onset. No comment was made regarding how these results should be applied in everyday clinical practice.

Randomised trial of efficacy and safety of inhaled zanamivir in treatment of influenza A and B virus infections
-
The Management of Influenza in the Southern Hemisphere Trialists Study Group: Randomised trial of efficacy and safety of inhaled zanamivir in treatment of influenza A and B virus infections. Lancet 1998, 352:1877-1881. A multicenter, randomized, double-blind, placebo-controlled trial conducted in Australia, New Zealand, and South Africa, involving 455 patients (321 with proven influenza). Zanamavir was given by inhalation only. Safety, tolerance and compliance were excellent. Efficacy was also demonstrated, but the thorny issues of the positioning of this therapy in management of influenza, and who should receive treatment are completely avoided.
