The fact that 57% of the 410 DSM-III-R primary, early-onset dysthymic patients reported family history of affective disorder in first degree relatives and that 51% of them had previous history of major depression also supports the strong relationship between dysthymic disorder and major depression. The most common comorbid axis I nonmood disorders were substance use disorder and panic disorder, and more than half of the dysthymic patients had comorbid Axis II personality disorders.

The authors investigated the platelet MAO activity in 58 patients with DSM-III-R dysthymic disorder and in 61 healthy control individuals. Female, but not male, patients showed significantly lower enzyme activity than (female) control individuals. The platelet MAO activity did not correlate with age, duration of illness and severity of dysthymic disorder, but female patients with history of suicide attempt were found to have the lowest enzyme activity.

If this was unsuccessfull, the patients were switched to a selective serotonin reuptake inhibitor for further 5-6 weeks. Treatment response was estimated at the end of the last drug trial.

The authors, having tested the mechanisms of action of hypericum extract in several well accepted biochemical models, found a weak inhibitory effect on MAO-A and MAO-B as well as clear reuptake inhibition of serotonin, noradrenaline and dopamine. In addition, subchronic trteatment resulted in significant down-regulation of beta receptors and significant upregulation of serotonin-2 receptors.
