Circulation 1998, 97:1382-1391. This experimental study shows that pathophysiologically relevant concentrations of TNF-α were sufficient to produce LV dysfunction and LV remodeling in rats. In this study, the increase in LV remodeling seemed to be caused by TNF-α-induced changes in the fibrillar collagen content in the rats.

J Cardiac Failure 1997, 3:117-124. This important experimental study shows that when TNF-α was selectively overexpressed in the hearts of transgenic mice, the mice developed LV dysfunction and LV remodeling and an increase in mortality compared with littermate controls.

Circulation 1998, 97:1375-1381. This study confirms that transgenic mice overexpressing TNF-α in the myocardium developed a dilated cardiomyopathy and increased mortality.

This interesting study showed that patients with advanced heart failure had increased circulating levels of CD14 (a marker of immune activation and possible exposure to circulating endotoxin) in their peripheral circulation. These results led the authors to suggest that the explanation for the increase in TNF-α levels in patients with chronic heart failure mesenteric venous congestion might be increased bowel permeability, bacterial translocation, and thereby endotoxin release.

In this study, no significant differences were found in the frequency of the TNF polymorphisms that have been associated with increased TNF production in patients with advanced heart failure and control subjects.

This study showed that pretreating myocardial slices of failing human myocardium (ex vivo) with adenosine was sufficient to prevent the endotoxin stimulated release of TNF-α from the failing myocardium. These studies raise the interesting possibility that adenosine might be used in the setting of heart failure to suppress TNF-α production in patients with human heart failure.

This important study showed that treating heart failure patients with pentoxifylline for 6 months led to an improvement in ejection fraction and survival and a decrease in peripheral TNF-α levels compared with patients treated with conventional therapy alone. Although this study did not prove that lowering TNF-α levels had a salutary effect on clinical outcome, it is the first to link decrease cytokine levels with an improved clinical outcome.
