N Engl J Med 1987; 316:1429-1435. Two-hundred and fifty-three patients with NYHA class IV heart failure symptoms were randomly assigned in a double-blind fashion to enalapril (2.5-40 mg/day) or placebo. Addition of enalapril to conventional therapy in patients with severe CHF can reduce mortality and improve symptoms.

Eight-hundred and four men receiving digoxin and diuretic therapy for heart failure were randomly assigned in a double-blind manner to receive 20 mg of enalapril daily or 300 mg of hydralazine plus 160 mg of isosorbide dinitrate. Enalapril produced a significant difference in mortality compared with hydralazine/isosorbide (18% versus 25% 2-year mortality, respectively).

Patients receiving conventional treatment for heart failure were randomly assigned to either placebo (n = 1284) or enalapril (n = 1285) at doses of 2.5-20 mg/day and then followed for an average of 41.4 months. The addition of enalapril to conventional therapy significantly reduced mortality and hospitalizations for heart failure in patients with chronic CHF and low ejection fractions.

N Engl J Med 1992; 327:685-691. Asymptomatic patients with ejection fractions less than 35% were randomly assigned to receive either placebo (n = 2117) or enalapril (n = 2111) at doses of 2.5-20 mg/day and were followed for 37.4 months on average. Enalapril significantly reduced the incidence of heart failure and the rate of related hospitalizations, compared with the rates in the group receiving placebo.

Patients with ejection fractions less than 40% without overt heart failure or symptoms of myocardial ischemia (n = 2231 ) were randomly assigned to placebo or captopril in a double blind fashion 3-16 days after myocardial infarction. In patients with asymptomatic left ventricular dysfunction after myocardial infarction, long-term administration of captopril was associated with an improvement in survival and reduced morbidity and mortality from major cardiovascular events.

This multicenter, randomized clinical trial of 19 394 patients was designed to assess the efficacy of lisinopril, transdermal glyceryl trinitrate, and their combination (over a 6-week treatment period) in improving survival and ventricular function after acute myocardial infarction. Treatment with lisinopril within 24 h of acute myocardial infarction produced significant reductions in overall mortality and in the combined outcome measure of mortality and severe ventricular dysfunction.

Lancet 1995; 345:669-685. In this study 58 050 patients were treated for 4 weeks after acute myocardial infarction with captopril (6.25 mg initially titrated to 50 mg twice daily) versus placebo. There was a significant reduction in 5-week mortaity which persisted for the long-term, and in those patients with previous myocardial infarction or heart failure the benefit seemed to be greater.
