In this paper, these authors make the point that in children with Crohn's disease with delayed skeletal maturity, Z scores for DXA need to be adjusted to height age or bone age to have a more accurate assessment of bone mineral density. In these patients, calculations based solely on chronologic age may overestimate the prevalence of osteopenia.

The authors of this study determined bone mineral density by DXA, and measured serum cytokine levels in 16 women both at the time of the diagnosis of celiac disease and then at a mean of 37 months on gluten-free diet. Patients with Z scores of less than -2 had significantly lower interleukin-1 receptor antagonist concentrations that those with Z scores of greater than -2. Interleukin-6 levels were inversely correlated with baseline lumbar spine Z score (R = -0.61; P < 0.01). The authors suggest that cytokines might have a role in the osteopenia associated with celiac disease.

Am J Gastroenterol 1999, 94:398-403. Bone density measured by DXA in lumbar spine and total body was compared between two groups: 1) 30 patients with celiac disease (age 21.9 y ± 4.4 y) on a strict gluten-free diet for a mean of 10.7 plus or minus 4.3 years (range, 5 y to 21 y) and 2) 240 healthy control subjects. In addition, intact parathyroid hormone, vitamin D levels, biochemical markers of bone formation (bone-specific alkaline phosphatase, N-terminal propeptide of type I procollagen in serum), and bone resorption (N-terminal telopeptide of type I collagen in urine) were also measured in these subjects. Bone mineral density in lumbar spine and total body, and all laboratory parameters tested, were not significantly different between celiac patients and control subjects after correction for confounding variables. Although these patients were not followed prospectively from the time of diagnosis, these data suggest that a strict gluten-free diet can reverse the decrease in bone mineral density associated with untreated celiac disease.

This is the most complete study to date on the effects of IBD on bone metabolism in children. It is a prospective study in 99 children with IBD and 63 healthy sibling control subjects, in which patients had an extensive clinical and laboratory evaluation and DXA to measure bone mineral density of the lumbar spine, femoral neck, and radius. Cumulative doses of corticosteroids were recorded. Bone mineral density Z scores were compared on the basis of bone age and pubertal stage in addition to chronologic age. Bone mineral density Z scores at the lumbar spine and femoral neck were lower in patients with IBD, and lower in those with Crohn's disease compared with those with ulcerative colitis. Low bone mineral density persisted after correction for bone age in girls with Crohn's disease. Cumulative corticosteroid dose was a predictor of low bone mineral density.
