Decreased catecholamine sensitivity and β-adrenergic receptor density in failing human hearts

Bristow MR, Ginsburg R, Minobe W, Cubicciotti R, Sageman WS, Lurie K, et al. Decreased catecholamine sensitivity and β-adrenergic receptor density in failing human hearts. New Engl J Med 1982; 307:205-211. This paper demonstrated that the myocardial β-adrenergic receptor system was defective in human CHF. β-Adrenergic receptor density and functional coupling were measured in samples taken from end-stage failing hearts at the time of explantation and it was found that β-adrenergic receptors were downregulated as much as 50% and adenylyl cyclase activities were also decreased. This was the first study demonstrating specific biochemical defects in β-adrenergic receptors in the failing human heart.

1-adrenergic receptors in the failing heart. Circulation 1993; 87:454-463. This study found that β-adrenergic receptor kinase 1 mRNA and kinase activity were significantly elevated in ventricular samples of end-stage failing human hearts. This explains the functional β-adrenergic receptor uncoupling seen in CHF.

In this study of transgenic mice, βARK1 and a peptide inhibitor of βARK1 were targeted to the myocardium. The authors demonstrated that overexpression of βARK1 could uncouple myocardial β-adrenergic receptor signaling and also attenuate cardiac functional responses to a β-agonist, while inhibition of myocardial βARK1 resulted in enhanced in-vivo performance. This study was the first to demonstrate in vivo that βARK1 could indeed desensitize β-adrenergic receptors and it also revealed that manipulation of βARK1 in the heart could have profound effects on myocardial performance.
