This study showed that MR imaging changes were common adjacent to inflamed synovial joints in patients with SpA but not rheumatoid arthritis. Furthermore, the enthesitis was often quite extensive and extended directly to the joint cavity, providing a possible explanation for the mechanism of synovitis.

This article emphasizes that a pathologic understanding of SpA will likely come from the evaluation of enthesitis lesions. It also suggests that inflammatory arthritis in general could be reclassified on the primary site of inflammation as synovial-based or entheseal-based.
