To test the hypothesis that the HFE gene, which had been identified as a candidate gene for hereditary hemochromatosis, was responsible for the disease, the effects of a targeted disruption of the murine homologue of the HFE gene were studied. HFE-deficient mice exhibit iron overload: HFE protein is involved in the regulation of iron homeostasis. The murine model provides opportunities to evaluate therapeutic strategies for the prevention or correction of iron overload.

Fleming MD, Trenor III CC, Su MA, Foernzler D, Beier DR, Dietrich WF, Andrew NC: Microcytic anaemia mice have a mutation in Nramp2, a candidate iron transporter gene. Nat Genet 1997, 16:383-386. Homozygous mk/mk mice have microcytic hypochromic anemia as a result of severe defects in intestinal absorption and erythroid iron utilization. The authors of this study have identified a strong candidate gene for mk and suggest that the phenotype is a consequence of a missense mutation in Nramp2, a previously identified gene of unknown function. This finding has implications for our understanding of iron transport.

This elegant review evaluates the practice of iron supplementation and emphasizes the importance of community involvement and a preventive orientation in iron supplementation programs. The authors develop the concept of "preventive supplementation," aimed at ensuring adequate iron nutrition throughout a woman's life cycle, and they propose the use of weekly doses, which are almost free of side effects and lead to better adherence than daily doses.

This controversial paper argues against the weekly administration of iron during pregnancy and favors the classic daily administration. Clear evidence that a mucosal block reduces the efficacy of daily iron therapy is lacking, and the side effects of daily doses have not been established on a scientific basis. A primary challenge in iron supplementation programs is to improve compliance.
