7 Nerlich AG, Schleicher ED, Boos N: 1997 Volvo award winner in basic science studies: immunologic markers for age-related changes of human lumbar intervertebral discs. Spine 1997, 22:2781-2795. This study correlates macroscopic with microscopic and immunochemical changes that start in the fetus and end in the ninth decade. The location of N-(carboxylmethyl)lysine, a marker of oxidative damage is correlated with progressive damage to disc structures. These findings have potential clinical implications for the protection of discs from an early age with anti-oxidant therapy.

Arch Phys Med Rehabil 1998, 79:366-374. This study confirms the importance of job satisfaction as a predictor of individuals with acute low back pain who will develop chronic pain. Prevention of chronic pain must include factors that improve work modification to improve job satisfaction.

26 Komori H, Okawa A, Haro H, Muneta T, Yamamoto H, Shinomiya K: Contrast-enhanced magnetic resonance imaging in conservative management of lumbar disc herniation. Spine 1998, 22:67-73. Internal enhancement of migrated portions of intervertebral hemiated discs was correlated with dissolution of the disc fragment and improvement in clinical symptoms without surgical intervention. Contrast-enhanced magnetic resonance imaging may become the diagnostic test of choice to identify patients who will resolve lower extremity radiculopathy with nonsurgical management.

-
29 Croft PR, Macfarlane J, Papgeorgiou AC, Thomas E, Silman AJ: Outcome of low back pain in general practice: a prospective study. Br Med J 1998, 316:1356-1359. At 1 year, 25% (42 of 170) patients had persistent low back pain. The general dictum of complete resolution in 2 months is overly optimistic. Acute low back pain must be treated aggressively to prevent chronicity.
