Alcohol Clin Exp Res 1997; 21:1320-1332. A complex design was employed, with numerous variables, and somewhat opaque results, some of which did not corroborate previous findings. The authors propose that males with multigenerational family history are over-represented in two subgroups of alcoholics: one characterized by enhanced sensitivity to alcohol reinforcement and the other characterized by high disinhibition (and probably high positive reward from alcohol) and alcohol-related problems.

In the year after hospital treatment for alcohol dependence, a diagnosis of current major depression at the time of hospitalization was associated with shorter time to first drink. There was no significant difference between women and men in this effect. Depressive symptoms recorded at monthly follow-up interviews on the Beck depression Inventory did not predict time to first drink or relapse in women or men.

Behav Genet 1997; 27:241-250. This is a large twin study. Genetic factors accounted completely for the association between depression and alcohol consumption in men, but accounted for only a part of the association in women. Note that the oldest individuals were only 26 years old, and alcohol dependence would not have had time to develop in the majority of at-risk individuals.

Patients admitted for treatment for alcohol dependence who were severely depressed recovered more from their depression, drank less and had more abstinent days if they took fluoxetine in a 3-month study [although rates of attaining complete abstinence were not significantly different (fluoxetine 28%, placebo 15%)].

This is a randomized controlled study of eight sessions of either relaxation therapy or CBT for depression in mild to moderately depressed alcohol-dependent patients also receiving outpatient and partial hospital treatment for alcoholism. The CBT group used Alcoholics Anonymous more and had better outcome in the 3 months after treatment finished. A good summary of CBT techniques is given. It is not clear whether the follow up was 'blind'.
