Specific cognitive features for somatization and for hypochondriasis are outlined. This study is fundamental for a cognitive view of somatization.

Interesting aspects of this study are the consideration of elderly, and the longitudinal design. The main focus is on the cascade from experiencing bodily changes, to the definition as illness and to subsequent illness behaviour.

Anxious hypochondriacal general practice attenders are compared with generally anxious and nonanxious patients in terms of symptom attribution. Anxiety and hypochondriasis could be distinguished on the basis of cognitive variables.

Cognitive and physical capacity process variables predict long-term outcome after treatment of chronic pain, J Consult Clin Psychol 1998; 66:434-439. Ninety-four chronic pain patients were followed up until 6 months after treatment. Cognitive and physical capacity changes during treatment made different contributions to long-term outcome.

Persons catastrophizing pain experiences had cognitive interference when expecting pain. This may be due to the amplification of somatosensory information, which limits the capacity for the processing of other external events.

The authors emphasize that illness anxiety is related to cognitive failures in everyday life and to a lesser extent to symptom reporting. Despite some interesting theoretical approaches, the examined sample is small and perhaps not representative (57 nonclinical individuals).

Prior pain experiences can influence the reactivity to external suggestion of pain. Although this is an interesting approach, some questions about the generalizability to somatization remains unclear.

Transition to chronic pain in men with low back pain: predictive relationships among pain intensity, disability, and depressive symptoms. Health Psychol 1998; 17:421-427. Functional disability plays a more prominent role than pain intensity in the transition from acute to chronic pain. The authors interpret their results using the conceptual framework of a 'failure to adapt'.

This paper represents an attempt to develop empirically based criteria for the classification of somatoform disorders. Data on 500 patients are used to analyse psychometric aspects of existing criteria. The impact of the study derives not just from the results, but also from the demonstration of a way in which classification criteria can be founded.
17. 17 Van Peski-Oosterbaan AS, Spinhoven P, Van der Does AJW, Bruschke AVG. Noncardiac chest pain: interest in a medical psychological treatment. J Psychosom Res 1998; 45:471-476. More than 1000 patients with noncardiac chest pain were assessed with regard to what extent they were interested in a medical psychological treatment. Predictors of interest in treatment were limitations in activity rather that pain characteristics. Of the patients studied, 64% indicated interest in psychological treatment.
18. 18 Hotopf M, Carr S, Mayou R, Wadsworth M, Wessely S. Why do children have chronic abdominal pain, and what happens to them when they grow up? Population based cohort study. Br Med J 1998; 316:1196-1200. This article is of major importance because of its longitudinal design and its extraordinary early establishment in 1946. The results confirm that parents' health is one of the most important predictors for physical symptoms in adulthood in children with abdominal pain.
19. 19 Herschbach P, Henrich G, von Rad M. Psychological factors in functional gastrointestinal disorders: characteristics of the disorder or of the illness behavior? Psychosom Med 1999; 61:148-153. Some psychological features found in patients with gastrointestinal disorders are not correlates of the symptomatology, but of illness behaviour. This study favours the distinction between both phenomena.
20. 20 Kessler D, Lloyd K, Lewis G, Gray DP. Cross sectional study of symptom attribution and recognition of depression and anxiety in primary care. Br Med J 1999; 318:436-440. A normalizing style of interpreting physical symptoms may sometimes be disadvantageous: physicians detected depression and anxiety less frequently when patients demonstrated this attributional style.

The study found different effects of burnout and perceived stress on hypothalamic-pituitary-adrenal axis regulation. Thus, this may present the key to conflicting results regarding cortisol in disorders associated with somatization.
