Absence or presence of a leak around tracheal tube may not affect postoperative croup in children
-
Khalil SN, Mankarious R, Campos C, Chuang AZ, Lemak NA. Absence or presence of a leak around tracheal tube may not affect postoperative croup in children. Paediatr Anaesth 1998; 8:393-396. An interesting study despite its limitations such as the small number of children with no leak around the endotracheal tube and only one anaesthesiologist being involved.

Comparison of cuffed and uncuffed endotracheal tubes in young children during general anesthesia

Khine HH, Corddry DH, Kettrick RG, Martin TM, McCloskey JJ, Rose JB, et al. Comparison of cuffed and uncuffed endotracheal tubes in young children during general anesthesia. Anesthesiology 1997; 86:627-631. The rather high rate of early respiratory complications in the children intubated with uncuffed tubes weakens the conclusions drawn from this otherwise well-conducted and challenging study.

The modified laryngeal mask airway: Four head and neck procedures in two children with mild subglottic stenosis
-
Rothschild MA, Kavee EH. The modified laryngeal mask airway: four head and neck procedures in two children with mild subglottic stenosis. Int J Pediatr Otorhinolaryngol 1997; 41:163-173. A trick to remember? In order to facilitate surgical access for ENT or ophthalmic surgery without using a reinforced LMA and a mouth gag, the authors adapted a malleable intubation stylet outside the LMA with adhesive tape: this allowed positioning of the breathing system away from the surgical field.

The modification of the endotracheal tube consists of cutting a U-shaped notch opposite to Murphy's eye in order to enlarge the bevel of a standard tracheal tube; this prevents blocking of the right upper lobe bronchus in a case of selective right mainstem bronchus intubation.

Assessment of oropharyngeal distance in children using magnetic resonance imaging
-
Greenberg RS, Zahurak M, Belden C, Tunkel DE. Assessment of oropharyngeal distance in children using magnetic resonance imaging. Anesth Analg 1998; 87:1048-1051. Reviewing 200 MRI head scans of children 0-17 years old, the authors measured the midline distance from the gums/teeth to the prevertebral space and from this line to the tip of the epiglottis. The oropharyngeal distance is related to age, weight and sex, and the distance to the epiglottis is highly variable.
