To our knowledge, this study is the first describing the findings related to the application of the NEP technique to patients with ILD. The authors compare 19 subjects with a variety of restrictive lung disorders (including 5 with various types of ILD) and 20 subjects with chronic obstructive airway disease. Nine subjects with chronic obstructive airway disease and only one subject with restrictive disease (scoliosis) demonstrated evidence of airflow limitation. Of all the subjects with expiratory flow limitation, inspection of the tidal V-V curve was not a reliable means of detecting the limitation.

Riley MS, Porszasz J, Miranda J, Engelen MPKJ, Brundage B, Wasserman K: Exhaled nitric oxide during exercise in primary pulmonary hypertension and pulmonary fibrosis. Chest 1997, 111:44-50. The authors prospectively studied nine patients with primary pulmonary hypertension, six patients with pulmonary fibrosis, and 20 normal volunteers. All subjects were studies at rest and during continuous incremental (ramp) cycle ergometry exercise and VNO was measured. In the pulmonary fibrosis group, VNO was significantly lower at rest and failed to rise in patients compared to normal controls. The authors suggest that the low resting VNO in pulmonary fibrosis may be due to loss of normal functional pulmonary capillary bed. The failure to increase VNO during exercise may reflect an inability to recruit the capillary bed.
