The authors found pulmonary hyperinflation in nine of 17 patients with Sjögren's syndrome. The higher the values of TLC and functional residual capacity, the lower were the maximum expired flow at 50% of vital capacity measurements and the more dyspnea occurred in patients. Such findings suggest the development of small airways disease, such as a mild proliferative bronchiolitis obliterans, in patients with certain vasculitides involving the lung, as also described in rheumatoid arthritis.

J Comput Assist Tomogr 1995, 19:924-931. In this prospective study, 27 patients with chronic disease (16 with IPF, 11 with COPD) who underwent HRCT were compared with 24 normal subjects. HCRT was performed at three degrees of lung inflation standardized by spirometry. Discriminatory power for lung density was best among all cohorts at mid-lung volume, which appeared to provide a compromise between compressive changes near respiratory volume and poor density discrimination near TLC.

Fifty-four patients with nonseminomatous tumor of the testis were randomly assigned to receive treatment with four cycles of bleomycin, etoposide, or cisplatin (27 patients) or with four cycles of etoposide or cisplatin (27 patients). VC and capillary blood volume decreased in patients receiving BEP but remained constant in those who received EP. By contrast, these authors found that the widely used DLCO was not a suitable parameter to monitor specifically pulmonary toxicity induced by bleomycin as part of a multidrug regimen. This is one of a few recent studies to describe the effects of antineoplastic agents such as bleomycin on the pulmonary endothelium rather than on alveolar cells and structure.

In this prospective study, 33 patients with locally advanced non-small-cell lung cancer underwent pulmonary function testing before, during, and after radiation and combined chemotherapy. No major short-term (at 2 and 6 months) functional effects of radiation alone on in combination with carboplatin were observed, especially of DLCO. Fixed reduction in membrane diffusing capacity and pulmonary capillary blood volume were attributed to obstruction of mediastinal vascular and lymphatic systems.

In the treatment of childhood brain tumors, a younger age at the time of the treatment with carmustine appears to be associated with a higher morbidity and mortality. Carmustine is associated with delayed pulmonary fibrosis. This study suggests the developing lung is sensitive to the harmful effects of carmustine.

Eur Respir J 1995, 8:1269-1274. In this prospective study, 43 patients underwent evaluation prior to, 3 months after, and 1 year after receiving bone marrow transplantation. In the majority of patients, lung volumes decreased by nearly 10% within 3 months but returned to baseline values 1 year later. DLCO dropped by 24% after 3 months and remained 13% below pretreatment levels after 1 year. Smoking contributed to more severe and persistent changes in the DLCO, suggesting that it sensitizes the lung for cytotoxic injury or that high-dose chemotherapy made the the lungs more susceptible to smoking-induced damage.

In this prospective nonrandomized study in a tertiary referral center, close to 1000 patients who had undergone bone marrow transplantation were investigated for the clinical significance of declines in pulmonary function 3 months after marrow transplantation. The authors found an increase in the nonrelapse mortality rate associated with either the presence of a restrictive defect 3 months after transplantation or a significant decline in lung volume compared with baseline. The effect was most prominent more than 1 year after transplantation and resulted from an increase in the rate of death with respiratory failure.

The authors found that patients with an increased or unchanged FVC and/or increased or unchanged DLCO after 1 year of treatment gain a statistically significant survival benefit over those with decreases in the same lung functions.
