This is a retrospective review of 192 out of 250 infants treated with vigabatrin as a first line drug for classical infantile spasms. Responses came within 7 days. Sixty-eight per cent showed an initial response, of which 21% relapsed and 22% had other seizure types later.

29. Neville BGR, Harkness W, Cross H, Cass H, Burch V, Lees J, Taylor D. Surgical treatment of severe autistic regression in childhood epilepsy. Pediatr Neurol 1997; 16:137-140. This paper describes a major syndrome of epileptic regression in two young children; one with a dysembryoplastic tumor and one without (Landau-Kleffner) right temporal syndrome. The regression involved cognition, language and social functioning. Both a useful but incomplete degree of recovery was achieved by surgical treatment (resection in one and subpial resection in the other).
30. Doose H, Neubauer B, Carlsson G. Children with benign focal sharp waves in the EEG-developmental disorders and epilepsy. Neuroped 1996; 27:227-241. A useful review is provided of the extensive literature on focal sharp waves and developmental disorders. It specifically examines the effect of most studies having started from either an electroencephalogram epilepsy position or a developmental perspective and attempts a synthesis.
31. Ferrie C, Marsden P, Maisey M, Robinson R, Cortical and subcortical glucose metabolism in childhood epileptic encephalopathies. J Neurol Neurosurg Psychiatry 1997; 63:181-187. This 2-fluoro-2-deoxyglucose positron emission tomography study of 32 children with epileptic encephalopathies (e.g. Lennox-Gastaut syndrome) reports diffuse cortical and subcortical hypometabolism. The limitations of control data and semiquantitative analysis are discussed in detail.
32. Ohtsuka Y, Murashima I, Asano T, Oka E, Ohtahara S. Partial seizures in West syndrome. Epilepsia 1996; 37:1060-1067. Of 92 children with West Syndrome 36 had partial seizures, sometimes with asymmetric spasma, hemiplegia and asymmetric hypsarrhythmia.
33. Duchowny M, Jayakar P, Harvey S, Resnick T, Alvarez A, Dean P, Levin B. Language cortex representation: effects of developmental versus acquired pathology. Ann Neurol 1996; 40:31-38. This epilepsy surgery study demonstrates that invasive recording can identify language cortex in children. In 12 out of 19 with a positive identification the language area overlapped or bordered the epilogenic region, particularly with dysplastic legions. Perinatal and postnatal lesions up to 5 years of age resulted in relocation of language but lesions occurring after this age did not.
34. Kuzniecky R, Gilliam F, Morawetz R, Faught E, Palmer C, Black L. Occipital lobe developmental malformations and epilepsy: clinical spectrum, treatment, and outcome. Epilepsia 1997; 38:175-181. Of 10 patients with occipital lobe developmental malformations, six showed specific visual ictal features. Only two had visual field defects suggesting some coexistence or relocation of visual function. Good results to surgical treatment were obtained.
35. Gadian D, Isaacs E, Cross J, Connelly A, Jackson GD, King MD, et al. Lateralization of brain function in childhood revealed by magnetic resonance spectroscopy. Neurol 1996; 46:974-977. This study of 22 right-handed children with temporal lobe epilepsy used H-magnetic resonance spectroscopy to assess the extend and laterlization of neuronal pathology. This group of children who were able to complete neuropsychological testing showed a correlation between left-sided abnormality and loss of verbal skills and right-sided abnormality and loss of nonverbal skills.
