Radical hysterectomy for stage 1B1 vs 1B2 carcinoma of the cervix: does the new staging system predict morbidity and survival? Gynecol Oncol 1985; 62:139-147. A total of 229 patients with stage IB cervical carcinoma treated with radical hysterectomy were assigned to the new FIGO sub-stages IB1 (n=181) and IB2 (n=48) based on clinical tumor diameter. Stage IB2 did have a significantly worse five-year survival rate (72.8%) when compared with stage IB1 (90.0%) (P=0.0265). Of the stage IB2 patients, 72% required adjuvant radiation therapy.

This technique employs larger fields in a four-field box arrangement, minimizing the possibility of missing the tumor or lymph nodes with the radiation fields. The four-year disease-specific survival rates were 76% for stage IIB and 53% for stage III. The grade 3 or higher rectal complication rate was 9%, while the overall grade 3 or higher complication rate was 14.8%.

Neoadjuvant 'quick VBC', three cycles of vincristine, bleomycin and cisplatin given 10 days apart, was followed by either RT alone or surgery+RT. The three-modality arm had a survival advantage that may be due to extended field RT in pathologically confirmed positive peri-aortic lymph nodes.

    • Chyle V, Zagars GK, Wheeler JA, Wharton JT, Delclos L. Definitive radiotherapy for carcinoma of the vagina: outcome and prognostic factors. Int J Radiation Oncol Biol Phys 1996; 35:891-905. This retrospective analysis of 301 patients with vaginal cancer treated at MD Anderson provides treatment parameters and outcomes with a median follow-up of 13 years. Tumor bulk and failure to achieve local control were independent determinants of metastatic relapse. The major determinants of local control were FIGO stage, tumor size, location (upper better than others) and circumferential involvement (posterior being worse). Non-clear cell adenocarcinoma was much worse prognostically than squamous cell carcinoma.
