Schreuder GM, et al.: Problems and possible solutions in finding an unrelated bone marrow donor: Results of consecutive searches for 240 Dutch patients. Bone Marrow Transplant 1997, 20:1011-1017. The search process, and obstacles encountered, for identifying a potential bone marrow donor for 240 Dutch patients is described. Those patients with haplotypes in high linkage disequilibrium were most likely to find a matched donor for all HLA loci studied. One third of the patients underwent transplantation, the reasons the remainder did not are given.

HLA-A, -B, and -DR gene and phenotypes of potential donors registered with the National Marrow Donor Program have been calculated and analyzed in relation to the major ethnic groups within the United States. These data emphasize HLA differences between different racial groups, with Caucasian, Latin, and Native Americans exhibiting less genetic diversity when compared with African and Asian Americans.

    • Natural mactivation of a common HLA allele (A*2402) has occurred on at least three separate occasions
    • Magor KE, Taylor EJ, Shen SY, Martinez-Naves E, Valiante NM, Wells RS, et al.: Natural mactivation of a common HLA allele (A*2402) has occurred on at least three separate occasions. J Immunol 1997, 158:5242-5250. The molecular defects in three HLA-A24 null alleles were found. Each differs from the common A*2402 allele by a single nucleotide: a substitution resulting in an in-frame stop codon, an insertion that alters the reading frame, and a substitution altering a splice site, which results in low expression of the A24 product. The clinical implications of HLA-null alleles for transplantation matching is discussed.

Blood 1997, 89:1818-1823. In a controlled clinical study, the association of mismatches at HLA-C loci, as determined by sequence analysis, with graft failure was demonstrated. This association was maintained when disparities not detected by serology for HLA-A and -B were considered. This study emphasizes the need to consider HLA-C matching in the selection of an unrelated bone marrow donor.

Sequence of two new Cw alleles (Cw*02023 and Cw*0707) and recognition by cytotoxic T lymphocytes. Tissue Antigens 1997, 49:612-623. Extensive analysis of HLA-C matching in groups of potential bone marrow donors and patients demonstrated considerable disparity for HLA-C (42%), and HLA-C was shown to be the target for CD8+ T cells from positive cytotoxic T lymphocyte precursor frequency assay. Analysis of linkage between HLA-A, -B and -C loci illustrates that matching for HLA-A and -B does not result in complete matching for HLA-C.
