This study analysed the long-term clinical outcome and survival after surgical revascularization for atherosclerotic RAS. A creatinine level of 3 mg/dl or less predicted favorable outcomes for blood pressure and renal function. Bilateral RAS also predicted a favorable renal function result.

This is a natural history study which reaffirms that atherosclerotic RAS is progressive. Stenosis of 60% or greater was associated with a 2-year atrophy rate of 21%. Atrophy correlated with declining renal function.

Am J Kidney Dis 1998; 32:611-622. This is a prospective study of 148 arteries treated by the endovascular stent for atherosclerotic, ostial RAS that had failed PTRA. The stent was highly successful (98%), and had low rates of re-stenosis and complications. Control of hypertension improved in the majority of cases, and renal function stabilized even in those with advanced renal failure. Four of eight dialysis-dependent patients recovered renal function.

The mortality rate of patients with bilateral RAS managed medically was high: 38% at 2 years. All were cardiovascular in nature. Rates of end-stage renal disease were 12% (crude) and 30% (adjusted for mortality) at 5 years. These rates appear higher than those reported for patients who undergo revascularization procedures. However, the actual impact on mortality and other major endpoints (end-stage renal disease and cardiovascular events) will be determined by prospective trials that compare treatment modalities.
