In this retrospective analysis of the prospective traumatic coma data bank series, the authors did not find a relationship between CT scan lesions and ALI, but this may be due to the limited resolution of CT in the posterior fossa. Patients with diffuse brain swelling and mid-line shift or low GCS should be considered at high risk of ALI.

Alveolar oedema fluid was examined after bronchoscopy and the protein content compared with plasma protein concentration and expressed as a ratio. Data from the largest series of patients with NPE, in whom pulmonary oedema fluid was examined for protein content, suggest that hydrostatic mechanisms are the common pathophysiological mechanism for neurogenic pulmonary oedema in humans.

In this study serum concentration of soluble sICAM-1 showed a strong relationship with outcome and was predictive if admission and 24 h measures were assessed. Neutrophil infiltration and cerebral oedema are related and a further pathophysiological mechanism may be mechanical obstruction or 'plugging' of the microvasculature as neutrophil adhesion and migration progresses.

Therapeutic moderate hypothermia was shown to cause a significant improvement in neurological outcomes 3, 6, and 12 months after injury for those patients with an initial GCS score of 5-7. The improvement in outcome for these patients was associated with a hypothermia-induced reduction of intracranial pressure and cerebrospinal fluid levels of IL-1β and glutamate.
