Am J Respir Crit Care Med 1997, 156:484-491. Reports the influence of iNO on the distribution of V/Q ratios throughout the lungs in experimental models of V/Q mismatch and shunt. These investigators used the multiple inert gas elimination technique to characterize the V/Q distribution and found that iNO may worsen oxygenation under some circumstances by relieving hypoxic pulmonary vasoconstriction.

This study was limited to patients with evidence of pulmonary hypertension. No difference in mortality was noted between the iNO-treated and control groups.
