The results indicate that low-dose aspirin is well tolerated and fully effective in inhibiting platelet PGG/H synthase also in subjects with high platelet count.

After the PVSG05 study (Tartaglia et al., Semin Hematol 1986, 23:172-176), this is the first randomized trial testing aspirin in polycythemic subjects. The results indicate that low-dose aspirin is well tolerated and fully effective in inhibiting platelet PGG/H synthase also in subjects with high platelet count.

A very important paper. It indicates that a laboratory tool may be useful for estimating the thrombotic risk in thrombocythemic patients. This hypothesis needs to be confirmed in a prospective study.
27. Najean Y, Rai JD: Treatment of polycythemia vera: the use of hydroxyurea •• and pipobroman in 292 patients under the age of 65 years. Blood 1997, 90:3370-3377. There is an extraordinary need for studies assessing the long-term effects of commonly used cytoreductive agents. This study provides a highly valuable contribution. The reported results, although not always easy to interpret, call further attention to the risks of hydroxyurea and pipobroman and provide basic information for further studies on this issue.

30. Regev A, Stark P, Blickstein D, Lahav M: Thrombotic complications in • essential thrombocythemia with relatively low platelet count. Am J Hematol 1997, 56:168-172. This paper reports the occurrence of microcirculatory disturbances in aspirin-treated patients. Although the authors use wide diagnostic criteria for the diagnosis of microcirculatory disturbances, the study raises the hypothesis that aspirin may not be uniformly effective in the treatment of these manifestations.

Thromb Haemost 1997, 78:1003-1007. This paper suggests that the development of aspirin resistance, as assessed by biological measurements, may result in clinical inefficacy of this drug. Aspirin resistance is a controversial phenomenon that may have a great clinical significance. This study is thus important, although the evidence of aspirin inefficacy is not fully convincing.
