The study reported in this paper shows that an inhibitor of ornithinine amino transferase, the enzyme that converts glutamine-derived pyrroline-5-carboxylate (P-5-C) to ornithinine, which in turn is the precursor of both citrulline and arginine, decreases plasma ornithine, citrulline and arginine concentrations of piglets by 59%, 52% and 76%, respectively. Because P-5-C synthase was located almost exclusively in enterocytes, the authors concluded that intestinal production of citrulline plays an important role in endogenous synthesis of arginine in piglets.

The objective of this study was to examine whole body leucine and urea kinetics in preterm infants receiving one of two parenteral amino acid intakes that differed primarily in branched-chain amino acid content. Rates of leucine appearance did not differ between groups but total leucine oxidation and the fraction of leucine oxidized were higher in infants receiving the mixture with more branched-chain amino acids. Interestingly, protein oxidation calculated from urea synthesis data did not differ; furthermore, there was no significant correlation between the rate of urea synthesis and the rate of leucine oxidation.

This prospective, randomized, double-blind trial of glutamine-supplemented versus unsupplemented parenteral nutrition regimens in premature infants confirms the safety of glutamine-supplemented regimens in such infants. In the smallest birthweight subgroup (birth weight <800g) supplemented infants required fewer days of parenteral nutrition (13 versus 21 days), achieved full feeds earlier (8 versus 14 days) and were weaned from ventilatory support sooner (38 versus 47 days).

    • 13C]-glucose was infused continuously for 4 h and appearance of labeled amino acids in plasma as well as the hepatically synthesized proteins, fibrinogen and VLDL apolipoprotein B-100 was monitored. As in previous studies [51], enrichment of plasma aspartate proline and cysteine was minimal, despite glucose precursor enrichment of 66%. Enriched aspartate and cysteine but not proline were bound to apolipoprotein B-100 reflecting intracellular compartmentation.
