In this review of antimicrobial susceptibilities of gram-negative bacilli isolated from 396 ICUs from 45 states between 1990 and 1993, resistance to third-generation cephalosporins was found to be an emerging problem. Risk factors for ceftazidime resistance were identified.

In this prospective study of invasive pneumococcal infections in Atlanta, Georgia from January through October 1994, of 431 isolates of S. pneumoniae, 25% were resistant to penicillin (7% were highly resistant). The rates of resistance to other antibiotics was also high, including trimethoprim/sulfamethoxazole (26%), erythromycin (15%), cefotaxime (9%), and multiple drugs (25%). Six serotypes (14, 6B, 9V, 23F, 19A, 6A) accounted for over 85% of isolates resistant to penicillin, cefotaxime, or multiple drugs. Thirteen of 15 with high-level resistance to penicillin and cefotaxime were serotype 23F.

This was a national surveillance study of 740 isolates of S. pneumoniae from 12 hospitals in 11 states during 1993 and 1994. Overall, 14% were penicillin nonsusceptible (MIC 0.1 μg/mL or more); an additional 3.2% were penicillin resistant (MIC 2 μg/mL or more). Penicillin resistance was higher in children younger than 6 years of age. Penicillin nonsusceptibility represented 15 serotypes, but 89% were in the 23-valent pneumococcal vaccine.

Clin Infect Dis 1996, 23:118-124. The rate of ceftazidime resistance among K. pneumoniae isolated from patients at one hospital increased from 6% to 28% between 1993 and 1994. Rates of resistance correlated with prior use of ceftazidime. Plasmid patterns suggest that most strains originated from a single clone. Ceftazidime resistance decreased following restricted use of ceftazidime and increased use of piperacillin/tazobactam.

This is a prospective study in an ICU where extended-spectrum β-lactamase-producing Enterobacteriaceae (ESBLPE) were endemic. The risk for acquiring ESBLPE increased during the ICU stay from 4% in the 1st week to 24% by the 4th week. Urinary catheterization and arterial cannulation were independent risk factors for acquiring ESBLPE. A single strain was responsible for most cases (by DNA macrorestriction analysis).

In this prospective study of resistance of S. pneumoniae isolated from hospitalized patients from a single hospital in Madrid, Spain, the incidence of erythromycin resistance (MIC greater than 1 μg/mL) increased from 7.6% in 1988 to 15.2% in 1992. Age less than 5 years and nosocomial acquisition were independent factors associated with erythromycin resistance. Ninety-four percent of macrolide-resistant strains displayed resistance to multiple antibiotics. Four of six patients with erythromycin-resistant strains were cured with erythromycin alone. Nine patients with pneumonia caused by erythromycin-resistant pneumococci were treated with ceftriaxone; all nine were cured.

During this 10-year prospective study of 504 adults with pneumococcal pneumonia in Barcelona, Spain, resistance to penicillin, cephalosporins, and erythromycin increased significantly. Overall, 29% of isolates were resistant to penicillin; 6% were resistant to ceftriaxone or cefotaxime. When other predictors of mortality were taken into account, mortality was no higher with penicillin-resistant or cephalosporin-resistant strains.

This study provides a description of a persistent outbreak of VRE and the control measures that were successful in decreasing the prevalence of vancomycin resistance. It provides the first evidence that vancomycin restriction can help reduce the prevalence of vancomycin-resistant organisms.

Infection control practices were not successful in limiting outbreaks of VRE at one tertiary care center. Restricting the use of cefotaxime, clindamycin, and vancomycin and adding extended-spectrum penicillins with β-lactamase inhibitors to replace third-generation cephalosporins were associated with a reduction in fecal colonization with VRE from 47% to 15%.
