A description of the epidemiologic characteristics of the first 500,000 cases of AIDS, highlighting the changes that have occurred since the epidemic began. The greatest proportionate increases in the incidence of AIDS are in blacks and Hispanics, and in the southern and midwestern United States. The proportion has increased among women, heterosexuals, and injecting drug users and has decreased in homosexual or bisexual men and among whites.

JAMA 1996, 276:105-110. This study validates the measurement of HIV-1 load in serum as a predictor of progression of HIV infection. From 1979 to 1995, serum specimen: from 165 persons with hemophilia and HIV-1 infection were stored. At ten years after sero-conversion, 72% of those who had 100,000 copies of HIV-1 RNA/mL, measured 12 to 36 months after seroconversion, had AIDS, compared with 52% in those with 10,000 to 99,999, 22% in those with 1000 to 9999, and 0% in those with fewer than 1000 copies of HIV-1 RNA/mL.

The percentages of deaths due to PCP, cryptococcosis, and candidiasis decreased, whereas there were increases in deaths due to nontuberculous mycobacteriosis, cytomegalovirus, bacterial septicemia, non-Hodgkin's lymphoma, tuberculosis, bacterial pneumonia, progressive multifocal leukoencephalopathy, cryptosporidiosis, and isosporiasis.

Twenty-one patients with PCP had BAL fluid analyzed with P. carinii-specific immunofluorescent monoclonal antibody reagent, and transbronchial biopsy specimens stained with Grocott methenamine silver nitrate. The yield of BAL was 95%, and the yield of transbronchial biopsy was 43%. Transbronchial biopsy was diagnostic for PCP in only one patient with a negative BAL.

In 35 randomized studies that involved more than 6500 patients and that compared different regimens of PCP prophylaxis and used an intention-to-treat analysis, TMP-SMZ was the best regimen at any of the doses given (one double-strength tablet daily, one double-strength tablet three times weekly and one single-strength tablet daily). Dapsone and aerosolized pentamidine were equally effective for PCP prophylaxis, and dapsone and TMP-SMZ were equally effective for toxoplasmosis prophylaxis. Dapsone, 100 mg daily, was more effective but not as well tolerated as 100 mg twice weekly. Pentamidine may be significantly more effective at a dose of 300 mg twice monthly rather than once monthly.

In 183 patients with tuberculosis and no history of HIV infection, 33 (18%) were found to be infected with HIV. At least one high-risk behavior for HIV infection was identified in 68% of the patients with tuberculosis, most often related to heterosexual contact (prostitute contact or multiple partners). Detailed study interviews identified risk factors in 30 of the 33 patients with HIV infection, but the medical record documented risk factors in less than half, which supports testing all patients with tuberculosis for HIV infection.

Am J Respir Crit Care Med 1996, 153:1982-1984. In a cohort of HIV-infected persons studied prospectively, the results of standard anergy testing were highly variable. Thirty percent of subjects initially classified as being anergic had positive reactions to mumps or Candida species antigens 1 year later. Cutaneous reactivity control antigens is also an unreliable indicator of a true negative tuberculin test because 39% of persons with a positive tuberculin test that later became negative (false-negative results) still reacted to mumps antigen. The authors suggest that the results of anergy testing should not be used to make clinical decisions on individual patients.

A retrospective comparison of computed tomography and conventional radiographic findings was performed between 42 HIV-seropositive and 42 HIV-seronegative patients with pulmonary tuberculosis. Intrathoracic adenopathy was significantly more common in the HIV-infected group on plain radiographs, but not on computed tomography. HIV-infected patients had a lower prevalence of consolidation, cavitation, and postprimary pattern on the computed tomography scans and a higher prevalence of miliary and extrapulmonary disease. Nodular opacities measuring 1 to 5 mm in diameter were the most common findings in both groups.

Infection was associated with a high mortality, but patients treated with drugs to which the isolates were susceptible showed improved survival.
